Here's a copy of the email message that recorded my answers to my sample
survey constructed at the University of Hawaii Ed Tech site in 1997 when I first
started online teaching and projects. There are many online survey services, if
you don't have a web page maker, such as Front Page, Dream Weaver, etc. Front Page reformatted the
print to fit the "theme" of this site, and I added the green to highlight the
answers that were typed into the form online.
